Thank you for entrusting us with your care. New patients are always welcome.

First Step

Review your Health Plan provider list to ensure that we are on the list. If you have questions about whether we are contracted to accept your health plan, contact us. We take most insurances.

As a new patient, you may complete and submit all 4 new patient forms from this website before your appointment. This will help expedite your visit.

Making an Appointment

Call our office in advance to make an appointment. Be sure to have your insurance card available.

When you arrive

Plan to arrive 20 minutes prior to your scheduled appointment to complete the necessary forms and insurance verification.

In the parking lot you will notice a golf cart valet that will take you to the front door if you want a ride. Amarillo Family Physicians Clinic is located on the first floor of the building.

Check in at the front desk where we will ask you to complete information, medical history forms, Consent to Treatment, and the Notice of Privacy Practices Acknowledgement forms, if you have not already completed these on our website. We will need your insurance card and drivers license, so be sure to have it with you. To make the most of your office visit, please bring the following:

  • Your insurance card
  • A government issued ID (Drivers license)
  • Your current medications (or an accurate list) including dose and frequency
  • Form of Payment

Payments and Your Health Plan

Our office policy requires that the fee for services be paid at the time of your visit. This includes co-pays, deductibles, and additional fees not ordinarily covered by insurance. Have your insurance card handy because we will need to verify it every visit. You may pay with:

  • Cash
  • Check
  • Mastercard, Visa, or Discover

We will file your claim to your health plan. Typically, your health plan will then send you an Explanation of Benefits or “EOB” detailing your processed claims and any amount that was not covered. Some health plans only make these statements available online, so be sure to check your health plan documentation.

If after your claims processes you still owe Amarillo Family Physicians Clinic a balance, we will send you an account statement by mail. You can pay your balance by:

  • Your patient portal
  • Website
  • Credit card over the phone
  • Mail a check to our office along with your statement
  • Payment in person

Privacy Policy

The relationship between you and your personal physician has always been a private confidential matter, and we recognize the trust you have placed in us to keep your information secure. We have an extensive written policy regarding the HIPAA privacy rules that you may review while you are here or take home. We will also ask you to acknowledge that we have told you about this.
