What to Bring to Your First Visit
- Photo ID (driver's license or state ID)
- Insurance card(s)
- List of current medications and dosages
- Medical records or history from your previous physician
- List of any allergies
- Completed new patient forms (available at the front desk)
What to Expect
Plan to arrive 15 minutes early to complete paperwork. Your first visit will include a comprehensive health assessment.
